Contraindications to GLP-1 Medications

If you have a personal or family history of medullary thyroid carcinoma (MTC) or
Multiple Endocrine Neoplasia syndrome type 2 (MEN 2), are pregnant or breastfeeding,
have a known allergy to semaglutide, or have active pancreatitis, you may not be a
candidate for GLP-1 medications. Severe gastrointestinal issues, severe kidney disease,
or gallbladder problems are also major warnings.

Key Contraindications

  • Thyroid Cancer/Tumors: Personal or family history of MTC or MEN 2.
  • Pregnancy/Breastfeeding: Potential harm to fetus or infant.
  • Allergies: Severe allergic reactions (anaphylaxis, angioedema) to semaglutide.
  • Pancreatitis: History of pancreatitis (inflammation of the pancreas).

Who Should Be Cautious:

  • Severe GI Issues: Severe gastroparesis, inflammatory bowel disease, or other serious stomach or intestinal conditions.
  • Kidney Problems: Not recommended for end-stage renal disease; may cause acute kidney injury.
  • Gallbladder Disease: Increased risk of gallstones (cholelithiasis) or gallbladder inflammation (cholecystitis).
  • Vision Changes: May worsen diabetic retinopathy.
  • Heart Issues: May increase heart rate; use caution with underlying heart disease.
  • Suicidal Thoughts: Wegovy (semaglutide for weight loss) carries a warning for suicidal ideation or behavior.
